Job overview
An exciting opportunity has arisen to work on H40 as a patient discharge assistant (PDA). You will be joining the Neurosurgical and Major Trauma Center Team to support with patient discharges.
We will provide a supportive training package to all candidates within a friendly nursing team.
We are looking for a dynamic and highly motivated candidate to support with complex discharges to work alongside the nurse in charge. The successful candidate will have excellent communication and negotiation skills, ability to prioritise workload, working proactively with the patient flow team and ward staff.
The candidate will be able to demonstrate excellent and accurate record keeping with effective IT and telephone skills.
Main duties of the job
The Patient Discharge Assistant will be expected to work as part of the ward team under the direction of the ward manager.
They will facilitate the discharge of patients from the ward in line with the Trust standards and policies.
The post holder will liaise and communicate effectively with relatives, community colleagues, other allied health professionals and medical / nursing staff.
Ensure that suitable patients are transferred to the appropriate hospital/community soon as possible.
Ensure that from admission they will collect collateral histories from family members. To ascertain whether patients are receiving the correct support and care in the home /
community.
The post holder will be expected to maintain confidentiality and adopt a positive attitude towards staff, patients and relatives at all times
